
SEED CERTIFICATION FORMS

· Application for Certification – This form will allow you to enroll a crop in the seed certification program. It can be used for classes of certified seed, as well as to enroll crops for phytosanitary inspections, quality assurance inspections, or other service inspections.
· Agreement for Certification – This form complimentary to the Application for Certification, as it states that we may come onto your property to perform inspections and that you will abide by the rules related to seed certification. This is a required form for all crops enrolled in the program.
· Seed Tag Request – Completing this form will provide all the information necessary to request tags from the Wyoming Seed Certification Service. Once completed, it can be emailed to Debbie Nuss at djnuss@uwyo.edu or faxed to 307-754-9820.
· Seed Blend Form – Certified seed lots can be blended. This form will allow you to document the seed used in a blend and thus request tags on the resulting seed lot.
· Seed Conditioner Report – Completing this form and providing it to the seed lab will assure that your sample is properly identified and that the Wyoming Seed Certification Service receives a copy of the seed test.
· Small Grain Land History – Small grains can have more than 6 fields on an application, but unfortunately, the Application for Certification only has room for the land history for 6 fields. This supplemental form will allow you to provide field history on additional fields.
· Wildland Collection Application – This is the pre-collection application for Source Identified wildland seed collections.
